A receipt will be emailed to you after the form is submitted. Please attach the receipt to the prize or write the submission number shown on the receipt on a piece of paper and attach to the prize when you send it in.

To be included in the program all prizes must be:
• Submitted using the form below by Friday, 22 May 2020
• Delivered to Giant Steps by Friday, 12 June 2020

The form below should only to be completed if the donation requires program acknowledgement and/or a thank you letter.

Multiple Lucky Dip Donations from the same donor only need to be entered once.

If no thank you letter or acknowledgement is required for Tombola Lucky Dip prizes, download a Tombola label and attach the label to the donation prior to delivering to Giant Steps.

Single Auction prices are usually valued at $200 and over. Smaller prizes may be put with something else or used in the Tombola lucky dip. If you don't know the value, just leave field empty.
The starting bid should be set at approximately 50% of the value. If no starting bid is requested, Giant Steps will set one. To get bidding started, Giant Steps may lower the requested starting bid.
Vouchers are required if there is no prize to hand over to the winner eg. accommodation, dinner, service, experience etc
If no, Giant Steps will prepare a voucher on their behalf.
